Job summary

Qualifications

• Master's degree in Mental Health & Behavioral Medicine or Social Work required.
• 2-3 years of clinical social work or mental/behavioral health experience required.
• Strong assessment, crisis intervention, and treatment planning skills with evidence-based interventions.

Responsibilities

• Provides psychosocial assessments of patients and families with social, emotional, interpersonal, and/or environmental issues.
• Formulates biopsychosocial assessment, disposition, and treatment plans.
• Provides mandated assessments when abuse is suspected and safety assessments when violence is suspected; plans discharge from care facility to home or other facility.
• Collaborates with other professionals to evaluate medical or physical condition and needs as part of the interdisciplinary team.
• Monitors, evaluates, and records client progress according to measurable goals in the treatment plan.
