Your Role
As a Behavioral Health Counselor at Near North Health, you will support patients’ mental and physical well-being through a collaborative care model. You will work closely with medical providers, psychiatric consultants, and the primary care team to provide evidence-based interventions, facilitate patient engagement, and monitor treatment progress. Your responsibilities include conducting assessments, offering brief therapeutic interventions, educating patients about mental health and substance use disorders, and coordinating care plans to achieve optimal patient outcomes. By fostering strong therapeutic relationships and providing compassionate care, you will help empower patients to achieve their mental health goals and improve their overall quality of life.
