Job summary

Open Health Care Clinic is seeking a Behavioral Health Counselor to conduct screenings, biopsychosocial assessments, and diagnostic evaluations, identifying needs and assigning DSM/ICD diagnoses. You will develop individualized treatment plans with evidence-based interventions and crisis support.

Coordination with multidisciplinary teams, referrals, and community resources ensures continuity of care while maintaining confidential documentation and contributing to staff training and compliance

Qualifications

• Active licensure in Louisiana (LCSW) or LPC required.
• Master's degree in social work or counseling required.
• Minimum 1 year of paid post-degree clinical experience preferred.
• Knowledge of trauma-informed care framework preferred.
• Knowledge of developmental disabilities and co‑occurring disorders preferred.
• BLS for healthcare providers certification required.

The Behavioral Health Counselor will conduct patient assessments, implement therapeutic interventions, complete individualized treatment plans, provide patient education, and complete referrals as necessary.

Job Description:

• Conduct patient screenings, biopsychosocial assessments, and diagnostic evaluations to identify needs and assign appropriate DSM/ICD diagnoses.
• Assess and address barriers to care while coordinating with multidisciplinary teams to support patient access to medical and behavioral health services.
• Develop, implement, and manage individualized treatment plans, providing evidence-based therapeutic interventions and crisis support as needed.
• Coordinate referrals, case conferences, and community resources to ensure continuity of care and patient engagement in treatment.
• Maintain accurate clinical documentation, confidential records, and required reports while participating in training, policy compliance, and behavioral health education initiatives.

Qualifications:

• Active LCSW with Louisiana State Board of Social Work Examiners or LPC with Licensed Professional Counselors Board of Examiners is required.
• Master's degree in social work or master's degree in counseling is required.
• At least one (1) year of paid post-degree experience in clinical experience providing mental health and substance use disorder services is preferred.
• Knowledge and experience with evidence-based practices, including trauma informed care framework.
• Knowledge and experience with developmental disabilities and children.
• Knowledge and experience working with co-occurring disorders (mental health and substance use) is preferred.
• BLS for healthcare provider is required.
