Job summary

Eventus WholeHealth seeks a Behavioral Health Support Specialist to assist residents in long-term care facilities under supervision of a licensed professional. Regional travel will be required as part of the role.

You will participate in assessments, care coordination with families and facility staff, and education for patients on self-management of their condition. Proficiency with electronic medical records and MS Office is preferred.

Qualifications

• Bachelor’s degree in Counseling, Social Work, Psychology or related field.
• Experience in a psychiatric or long-term care setting preferred.
• Customer service experience preferred.
• Ability and willingness to function independently and proactively in a long-term care setting.
• Effective use of electronic medical records and Microsoft software (Teams, Excel, etc).
• Skilled in written and verbal communication.
• Skilled in exercising a high degree of initiative, judgment and discretion.
• Ability to handle confidential and sensitive information.

Responsibilities

• Conduct assessments that include the behavioral and social needs of the patients
• Meet with families and facility staff to coordinate care as recommended by licensed professionals
• Assess the patient’s knowledge of their clinical condition and provide education and self-management support
• Provide outreach to clients who are having problems with treatment adherence
• Assist in the development of the individual goals and behavioral health treatment plan
• Coordinate with other agencies and community providers
• Maintain timely documentation of work
• Serve as an active member of the behavioral health treatment team
