Job summary

Extended Care Specialists INC seeks a Behavioral Health Support Specialist to assist residents of long-term care facilities under supervision of a licensed professional. Regional travel is required.

You will conduct assessments, educate families and staff, support treatment adherence, help develop individualized goals, and document activities as part of the behavioral health team. Strong communication and ability to work independently in a facility setting are essential.

Qualifications

• Bachelor’s degree in Counseling, Social Work, Psychology or related field.
• Experience in a psychiatric or long term care setting preferred.
• Customer service experience preferred.
• Ability to work independently and proactively in a long term care setting.
• Effective use of electronic medical records and Microsoft software (Teams, Excel, etc).
• Skilled in written and verbal communication.
• Skilled in exercising a high degree of initiative, judgment and discretion.
• Ability to handle confidential and sensitive information.

Responsibilities

• Conduct assessments that include the behavioral and social needs of the patients.
• Meet with families and facility staff to coordinate care as recommended by licensed professionals.
• Assess the patient’s knowledge of their clinical condition and provide education and self-management support.
• Provide outreach to clients who are having problems with treatment adherence.
• Assist in the development of the individual goals and behavioral health treatment plan.
• Coordinate with other agencies and community providers.
• Maintain timely documentation of work.
• Serve as an active member of the behavioral health treatment team.
