Job summary

Extended Care Specialists INC is seeking a behavioral health support professional to assist residents of long term care facilities under the supervision of a licensed professional. This role may include training in assessments and interventions aligned with individual care plans, with regional travel required.

Qualifications

• Bachelor's degree in Counseling, Social Work, Psychology or related field.
• Experience in a psychiatric or long term care setting preferred.
• Ability and willingness to function independently and proactively in a long term care setting.
• Proficient in electronic medical records and Microsoft Office (teams, excel, etc).
• Strong written and verbal communication.
• Ability to handle confidential and sensitive information.

Responsibilities

• Conduct assessments that include the behavioral and social needs of the patients
• Meet with families and facility staff to coordinate care as recommended by licensed professionals
• Assess the patient's knowledge of their clinical condition and provide education and self-management support
• Provide outreach to clients who are having problems with treatment adherence
• Assist in the development of the individual goals and behavioral health treatment plan
• Coordinate with other agencies and community providers
• Maintain timely documentation of work
• Serve as an active member of the behavioral health treatment team
