Job summary

Jefferson Health Plans seeks a Behavioral Health Consultant (BHC) to deliver PCBH services in ambulatory practices across specialties. The role supports PCPs with assessment, brief interventions, and care planning for patients and families, from toddlers to older adults.

Licensure as LCSW, LPC, or LP in PA is required, with at least one year in a behavioral health setting. Collaboration with research teams and integration into primary care are expected.

Qualifications

• Master’s Degree in Counseling or Social Work from an accredited program.
• Doctorate (PhD/PsyD) in Psychology from an accredited university.
• 1 year of experience in a behavioral health setting.
• Knowledge of medical terminology and documentation best-practices.
• Ability to work with multidisciplinary teams and diverse patient populations.
• Licensure: LCSW, LPC, or LP in PA.

Responsibilities

• Interact with patients and families to deliver PCBH services within ambulatory practices.
• Provide assessments, brief interventions, and care planning alongside PCPs.
• Collaborate with research teams as needed to support program goals.
• Participate in training and education for patients and staff.
• Triaging referral to specialty mental health care when appropriate.
• Support culturally appropriate care in line with state laws.
