Job summary

Cabarrus Rowan Community Health Centers is seeking a Behavioral Health Clinician for Special Populations. You will promote, provide, and expand behavioral health and substance abuse services, focusing on patients with high clinical complexity and social determinants impacting health.

You will conduct screenings, diagnose and treat, coordinate with a team of care providers, and offer educational interventions to improve access to care.

Qualifications

• Ability to assess, treat, prevent, and provide strategies and care planning for patients with chronic health, mental health, behavioral health, and substance use conditions.
• Experience with motivational interviewing and behavioral health modalities, particularly transtheoretical model.
• Ability to interact with diverse populations respectfully and professionally.

Responsibilities

• Facilitate screenings and provide treatment and referrals within an integrated care team.
• Administer behavioral health and wellness assessments (e.g., PHQ-9) and provide evidence-based interventions.
• Address social determinants impacting health outcomes and coordinate with other services.
• Identify, assess, and facilitate access to substance abuse and behavioral health services for special populations.
• Provide educational interventions supporting chronic disease management and behavioral health.
• Develop and review care plans with shared decision making.
• Participate in workgroups/trainings and integrate best practices in clinical work.
• Contribute to quality improvement activities and program goals.
• Support regulatory and certification requirements through documentation and initiatives.
• Perform other tasks as assigned.
