Job summary

Health Care Partners of South Carolina is seeking a Behavioral Health Provider to deliver comprehensive behavioral health services within an FQHC setting. You will perform assessments, provide individual counseling, crisis intervention, and treatment planning across the lifespan.

Collaborating with primary care, behavioral health, nursing, pharmacy, and other teams, you will develop and monitor treatment plans, coordinate care, and ensure HIPAA compliance while utilizing the EHR for

Qualifications

• Master’s degree in Social Work, Counseling, Psychology, Marriage and Family Therapy, or a related behavioral health field.
• Current South Carolina licensure as LISW-CP, LPC, LMFT, or LAC.
• Minimum 1 year of outpatient behavioral health counseling experience preferred.
• Experience in FQHC or community health center preferred.
• Experience using EHR systems preferred.

Responsibilities

• Provide behavioral health assessments, diagnostic evaluations, individual counseling, crisis intervention, and treatment planning.
• Develop, implement, and monitor individualized treatment plans based on patient needs and evidence-based practices.
• Coordinate care with primary care providers, behavioral health staff, nursing, pharmacy, dental, and other interdisciplinary team members.
• Refer patients to community resources, specialty providers, inpatient treatment, and other support services as clinically appropriate.
• Participate in integrated care initiatives that promote whole-person health and improve patient outcomes.
• Maintain accurate, timely documentation within the Electronic Health Record (EHR).
• Monitor patient progress, evaluate treatment effectiveness, and modify treatment plans as appropriate.
• Maintain compliance with HIPAA, HRSA, FQHC standards, organizational policies, and applicable state and federal regulations.
• Participate in quality improvement initiatives, provider meetings, continuing education, and departmental activities.
• Establish and maintain collaborative relationships with community agencies and referral partners to improve access to behavioral health services.
• Provide patient and family education regarding behavioral health conditions, treatment options, coping strategies, and community resources.
• Perform other duties as assigned.
