Job summary

Cabarrus Rowan Community Health Centers is seeking a Behavioral Health Clinician – Special Populations to promote and expand behavioral health and substance use services within high-need communities. The role emphasizes patient assessment, treatment, and coordination with a multidisciplinary team across multiple clinic sites.

The incumbent will provide screenings, brief interventions, and care navigation, with emphasis on vulnerable populations.

Qualifications

• 2 years professional experience in primary care, behavioral health, or community-based setting.
• Experience with behavioral health screening, social work, or related work.
• Competence in diagnostic formulation across complex presentations.
• Knowledge of legal and ethical standards for the delivery of behavioral health care.
• Ability to navigate multi-system collaboration (healthcare, education, justice, housing, social services).

Responsibilities

• Support the integrated care team with screenings, providing treatment, under supervision, and referrals to appropriate support services.
• Administer behavioral health, substance abuse, or wellness assessments, including PHQ-9 and provide treatment using evidence-based interventions.
• Assess and intervene with social determinants that may interfere with health outcomes.
• Identify, assess, treat, refer, and promote services tailored to the special population.
• Provide educational interventions for behavioral health and chronic disease management.
• Develop, review, and evaluate patient care plans through shared decision making.
• Participate in quality improvement activities and trainings specific to special populations.
