Qualifications

• Master’s degree in social work or related field.

• Excellent written and verbal communication skills.

Responsibilities

• Diagnoses and treats mental, emotional, and behavioral disorders; modify care plans as patient status changes.
• Refer patients to other services to improve well‑being (community services, treatment programs, support groups, psychiatrists).
• Develop discharge plans with multi‑disciplinary teams to ensure continuity of care.
• Perform psychosocial assessments and provide crisis, bereavement, and other behavioral health interventions as needed.
• Educate and coordinate with patient care teams and outside agencies to ensure timely discharge and quality care.
