River Valley Family Health Centers in Montrose, CO is seeking a licensed Behavioral Health Provider to deliver integrated behavioral health services within a primary care clinic. You will perform intakes, assessments, develop treatment plans, provide crisis intervention, and coordinate care with MAT services and the medical team.

Qualifications

• Licensed social work or counseling licensure in Colorado.
• Good written documentation and EMR skills.

Responsibilities

• Performing intakes, assessments, and developing treatment plans for patients.
• Providing family treatment, crisis intervention, short-term, and outpatient psychotherapy within the clinic.
• Monitoring patient progress and documenting in the EMR.
• Collaborating with an interdisciplinary team and coordinating with MAT services.
