Job summary

Hennepin Healthcare in Minneapolis is seeking a Licensed Mental Health Professional to join the Collaborative Care Model team as a Behavioral Health Clinician. This full-time, on-site role works days in the downtown campus, with a five-year SAMHSA grant supporting the program.

The BHC coordinates behavioral health care with PCPs and a psychiatric consultant, delivering brief psychotherapy, treatment planning, and measurement of response.

Qualifications

• Must be a licensed Mental Health Professional per MN statute (LMFT, LICSW, or LPCC).
• Meets criteria for 'Mental Health Professional' as defined in MN statute.
• Grant-funded position through SAMHSA five-year grant (through 2028).

Responsibilities

• Participates in multidisciplinary care team rounds
• Provides mental health services including screening, diagnostic assessment, treatment planning, psychotherapy, crisis services, referrals, and discharge planning
• Completes documentation for accurate and timely billing of clinical services
• Provides psychoeducation to patients, families, and community organizations as needed
• Provides care coordination including discharge planning
• Provides on-call phone coverage for patient coaching
• May act as a preceptor to interns and students
• Assists in maintaining accreditation standards and licensure compliance
• Provides clinical supervision to graduate-level practitioners in accordance with guidelines
• Other duties as assigned
