Job summary

Optum, in partnership with Summit Medical Group, seeks a Behavioral Health Clinician to deliver care within a primary care setting under the Collaborative Care Model (CoCM). You’ll work with a multidisciplinary team, providing evidence-based behavioral health services to adults and adolescents in a hybrid schedule.

The role emphasizes brief interventions, measurement-based care, and collaboration with primary care and psychiatric consultants.

Qualifications

• Active Tennessee behavioral health license (LCSW, LMSW, LPC-MHSP, LMFT)
• 1+ year of behavioral health treatment experience
• Experience assessing and treating common mental health conditions
• Proven comfort delivering brief, evidence-based interventions
• Proven interest in multidisciplinary, team-based care model
• Comfortable working with diverse patient populations across mental health and medical conditions

Responsibilities

• Provide brief, evidence-based behavioral health interventions in person and via telehealth
• Screen, assess, and diagnose common mental health and substance use disorders
• Collaborate with primary care providers, psychiatric consultants, and care team members
• Track patient progress using measurement-based care and adjust treatment as needed
• Support psychotropic medication management by monitoring adherence and side effects
• Participate in regular psychiatric case consultations and communicate recommendations
• Develop relapse prevention and self-management plans for discharge from active care
• Maintain accurate documentation and follow collaborative care workflows
