Job summary

Qualifications

Responsibilities

• Screen telephone referrals.
• Perform psychosocial evaluation and assess mental health service needs of patients and families.
• Develop and implement short-term service plans with the program team.
• Provide counseling, care management and referrals for long-term treatment.
• Coordinate and follow up on linkages with other service agencies to ensure continuity of care.
