Chimes is seeking a Case Manager to provide community-based behavioral health services in Newark, DE. You will assess resources to help clients live safely in the community and maintain independence, while coordinating with families, payers, and community supports.

The role requires handling up to 30 clients, developing strength-based plans, and ensuring timely documentation. The position follows a recovery-informed approach, with a Monday–Friday schedule, and a 24/7 on-call rotation to meet

Formación

• Bachelor’s degree or RN license or HS diploma with social services experience
• Experience in public or private human services with direct client contact
• Knowledge of Recovery Model principles

Responsabilidades

• Carry a caseload of up to 30 clients
• Perform case management duties for consumers in Behavioral Health Services
• Develop individualized, strength-based service plans with measurable outcomes
• Assist consumers in obtaining housing, food, healthcare, employment, and socialization
• Provide crisis assessment and intervention as needed
• Coordinate communication among service providers and support systems
• Submit payroll and billing documentation in a timely manner
• Be available for 24/7 on-call rotation
