Quadrant Health Group’s Seaside Wellness of Palm Beach is seeking a Behavioral Health Case Manager to support residents through admission to discharge in Palm Beach Shores, FL. You will assess needs, develop individualized care plans, and coordinate services to sustain recovery.

The role emphasizes collaboration with clinicians, family outreach, and treatment planning, with a focus on ethical standards and compassionate care. A valid Florida driver's license is required.

Qualifikationen

• Bachelor's degree in social work, psychology, counseling, or related field; Master’s preferred.
• Minimum of 3 years in behavioral health case management.
• Florida Driver's license is required.
• Experience with diverse populations and electronic health records (EHR).
• CPR and First Aid certifications required.

Aufgaben

• Conduct comprehensive assessments to identify client needs and develop individualized care plans.
• Coordinate behavioral health services including therapy, medication management, and support groups.
• Connect clients with housing, food assistance, employment services, and transportation.
• Develop Comprehensive Treatment Plan with input from the client and participation in planning meetings.
• Advocate for clients' rights with healthcare providers and social service agencies.
• Provide crisis intervention and maintain documentation of client progress.
• Collaborate with therapists, psychiatrists, and other healthcare professionals.
• Communicate with family members to arrange appointments and update on progress.
