Marina Harbor Detox, part of Quadrant Health Group, is seeking a passionate Behavioral Health Case Manager to support residents with mental health and/or substance use disorders in San Francisco. You will guide admission through discharge, develop individualized treatment plans, and coordinate services.

The role emphasizes collaboration with therapists and physicians, thorough documentation, and adherence to treatment planning.

Qualifikationen

• Bachelor's degree in social work, psychology, counseling, or a related field.
• Minimum of 3 years of experience in behavioral health case management.
• Experience working with diverse populations.
• Proficiency in electronic health records (EHR) systems.
• Current CPR and First Aid Certification.
• Successful completion of Pre-Employment Requirements including criminal background clearance, drug testing, and health screening.

Aufgaben

• Conduct comprehensive assessments to identify client needs and develop individualized care plans.
• Coordinate and monitor the delivery of behavioral health services, including therapy, medication management, and support groups.
• Connect clients with community resources, such as housing, food assistance, employment services, and transportation.
• Develops the Comprehensive Treatment Plan with client input and participation in planning meetings.
• Advocate for clients' rights and needs with healthcare providers and social service agencies.
• Provide crisis intervention and support as needed.
• Maintain accurate and timely documentation of client interactions and progress.
• Collaborate with interdisciplinary team members, including therapists and psychiatrists.
