Job summary

Quadrant Health Group is seeking a dedicated Behavioral Health Case Manager to support residents through admission to discharge at Ocean Coast Recovery Center, Orange County, CA.

As a Full Time Float Case Manager, you will assess client needs, coordinate therapy and supports, connect with community resources, and develop individualized treatment plans to promote recovery and stability for individuals with mental health and/or substance use disorders.

Qualifications

• Experience in behavioral health case management.
• Knowledge of DHCS, JCAHO accreditation, and regulatory standards.
• Strong understanding of community resources and how to access services.
• Ability to document client progress and develop treatment plans.

Responsibilities

• Conduct comprehensive assessments to identify client needs and develop care plans.
• Coordinate therapy, medication management, and support groups.
• Connect clients with housing, food assistance, employment services, and transportation.
• Develops Comprehensive Treatment Plans with client input.
• Advocate for clients with healthcare and social service agencies.
• Provide crisis intervention and support as needed.
• Maintain accurate documentation of client interactions and progress.
• Collaborate with therapists, psychiatrists, and other professionals.
• Communicate with residents' family members about status and progress.
• Monitor progress and adjust care plans as needed.
