Job summary

Fallon Health is seeking a Behavioral Health Case Manager (BHCM) to assess members’ behavioral and social care needs, develop and implement care plans, and collaborate with the care team. The BHCM supports access to outpatient services, community resources, and government programs.

The role requires a Master’s degree in social work or related field, LICSW/LMHC license, and at least three years of behavioral health experience. Excellent communication and documentation skills are essential.

Qualifications

• Master’s degree in social work or related field required.
• Active LICSW or LMHC license; CORI required.
• Minimum 3 years clinical behavioral health experience.
• Strong collaboration with interdisciplinary care teams.

Responsibilities

• Assess members’ behavioral and social care needs and develop individualized care plans.
• Coordinate care with Fallon Health Care Team and community resources.
• Document outcomes and referrals in the care documentation system.
• Support supervision and participate in team meetings and rounds.
• Assist members with access to government programs and resources.
