Job summary

Chrysalis Health is seeking a CAT Team Behavioral Health Case Manager to support youth and families across Florida in community-based settings. This role emphasizes hands-on case management with weekly home or community visits and close collaboration with a multidisciplinary team.

The ideal candidate holds a Bachelor’s in psychology, social work, mental health counseling, or related field and has 1+ year working with youth with behavioral health needs, plus a valid Florida driver’s license.

Qualifications

• Bachelor’s degree fields as listed (psychology, social work, MHC, MFMT or related).
• Minimum 1 year of experience with youth with behavioral health populations.
• If you hold a Bachelor’s in another field, you need 3 years of relevant youth mental health experience.
• Valid Florida driver’s license, reliable vehicle, and good driving record.
• Ability to pass Level 2 background screening.

Responsibilities

• Provide hands-on, community-based case management with youth and families.
• Meet weekly with youth and families in home or community settings.
• Collaborate with therapists, nurses, mentors and psychiatrists.
• Develop individualized service plans and connect clients to healthcare, education, housing, and employment resources.
• Coordinate care with families, providers, and community partners.
• Advocate for client stability and independence.
• Assist with transportation to appointments, court, or services.
• Monitor progress and adjust plans as needed.
• Complete documentation timely and in compliance.
