Job summary

Children's Home Society of Florida (CHS) is seeking a dedicated Targeted Case Manager to assess client needs, coordinate services, and advocate across schools and health providers. You will work in homes, schools, and community settings to help families move from crisis to stability.

Required: Bachelor's in Human Services or related field, AHCA Targeted Case Management certification within 3 months, Florida driver's license, 1 year working with emotionally disturbed children (or 3 years if no

Qualifications

• Bachelor's Degree required in Human Services/Social Work or related field.
• Minimum 1 year working with severely emotionally disturbed children (or 3 years if degree is outside Human Services).
• Florida Driver's License with access to a reliable, insured vehicle.
• Can work Mon-Fri, 8-5pm, with flexibility to work outside standard hours when needed.
• Completion of AHCA-approved Targeted Case Management certification training within three months of hire.

Responsibilities

• Conduct comprehensive assessments to identify client and family needs and develop individualized service plans that drive meaningful outcomes.
• Connect children and families to critical community resources and treatment services.
• Advocate for clients across schools, healthcare systems, behavioral health providers, and community organizations.
• Provide in-home support and guidance to families.
• Document progress and outcomes through our electronic health record system.
• Participate in family service planning, case reviews, and community outreach efforts.
