Behavioral Aid Solutions is seeking a Certified Behavioral Health Case Manager to provide Medicaid Targeted Case Management for children and/or adults across Florida. You will conduct needs assessments, develop service plans, and coordinate access to care, school services, and social supports through home, school, and community visits.

The role requires a Bachelor's degree and active CBHCM/CBHCM-P certification, with 1 year of experience preferred.

Qualifications

• Bachelor's degree required.
• Active Florida CBHCM or CBHCM-P certification required.
• 1 year of direct behavioral health case management experience preferred.
• Familiarity with Medicaid documentation requirements.

Responsabilités

• Conduct needs assessments and develop individualized case management/service plans.
• Coordinate client access to resources, medical/psychiatric care, school services, and social services.
• Provide case management in home, school, and community.
• Monitor progress toward goals and adjust service plans as needed.
• Maintain Medicaid-compliant documentation of every encounter.
• Collaborate with treating clinicians, families, schools, and community agencies.
