Job summary

You will promote wellness, coordinate with treatment teams, conduct home visits, and ensure documentation standards while collaborating with community partners to support recovery-oriented care.

Qualifications

• Must meet one of the education and experience combinations identified by the position requirements.
• Valid California Driver License with an insurable driving record.
• Behavioral health experience preferred.
• Knowledge of community resources preferred.
• Experience working with adults with mental health or co-occurring disorders preferred.
