Community Service Board of Middle Georgia is seeking a PSR-I Case Manager in Eastman, GA. The role supports Emerging Adult clients, adheres to treatment plans, and documents progress to help individuals access needed services.

The entry-level position requires a high school diploma or GED, a valid GA driver’s license, and strong communication skills. Experience with Microsoft 365 is preferred, with opportunities for growth within the program.

Qualifications

• High school diploma or GED required.
• Valid Georgia driver’s license.
• Strong verbal and written communication skills.
• Ability to work independently and with others; good interpersonal skills.
• Experience using Microsoft 365 Office products.

Responsibilities

• Make required contacts with consumers each month; minimum twice daily contacts as needed.
• Adhere to treatment plans and write progress notes; bill for services.
• Follow DBHDD guidelines and meet documentation timelines.
• Identify consumer needs and refer to appropriate services.
• Provide information on outside resources and encourage independent access.
• Communicate professionally; support teamwork and constructive feedback.
