• Strong interpersonal skills and ability to communicate with clients, families and professionals.
• Knowledge of behavioral health practices and community resources.
• Ability to work independently and as part of a team.

Responsibilities

• Coordinate and provide care that is safe, timely, effective, efficient, and client-centered.
• Manage case assignments, draft service plans, and review case progress regularly.
• Advocate for clients, assisting them in making informed decisions regarding their clinical status and treatment options.
• Collaborate with therapists, clients, and family members to ensure all client behavioral health needs are met.
• Facilitate clients in achieving personal independence and develop daily living skills.
• Upholds documentation and billing practices per CARF, CMS, and training.
