Job summary

Louisiana Blue seeks a Behavioral Health Case Manager to coordinate and manage behavioral health services for members enrolled in Medicare and Commercial plans. You will perform comprehensive assessments, develop care plans, coordinate services across inpatient, outpatient, and community settings, and monitor progress to improve health outcomes.

The role includes crisis intervention, risk assessment, and immediate escalation of emergencies, collaboration with medical and behavioral health

Qualifications

• Bachelor’s degree or higher in a relevant field.
• RN/LPN/LPC/LCSW/LN license required or equivalent.
• Minimum 3 years of case management experience in behavioral health.
• Experience with crisis intervention and behavioral health care coordination.
• Proficiency with electronic health records and case management systems.

Responsibilities

• Conduct comprehensive behavioral health assessments to identify needs and goals.
• Develop, implement, and monitor individualized care plans with members and providers.
• Coordinate behavioral health services across inpatient, outpatient, and community settings.
• Facilitate level of care transitions and ensure follow-up after hospitalizations or emergencies.
• Document all activities in compliance with standards and participate in quality improvement.
