Qualifications

• Bachelor's degree in social work or a clinical mental health field.
• Two years' experience in a mental health care facility preferred; must have strong communication skills.
• CPR certification and HANDLE WITH CARE training within 90 days of employment and prior to restraining procedures.

Responsibilities

• The Case Manager assists with social services functions including communication with referral sources, families, physicians, and schools, ensuring patients/families are scheduled aftercare, short-term and long-term family therapy, providing information and referrals as needed and conducting wellness checks.
