Responsibilities

• Complete case management of all patient needs.
• Interview patients and families to assess needs and risk, and develop plans of care.
• Coordinate services and collaborate with doctors on plan of care.
• Implement, monitor and evaluate the plan of care and its effectiveness.
• Communicate regularly with staff and family on patient progress.
