Job Summary

The Case Manager is responsible for complete case management of all patient needs.

  • Interview patients and families to assess needs and risk categories to develop and implement appropriate plans of care.
  • Ensure quality clinical care by coordinating all services and collaborating with doctors on the plan of care.
  • Implement, monitor, and evaluate the plan of care and its effectiveness on a regular basis.
  • Communicate regularly with relevant staff members and family members on care progress.
Essential Job Functions
  • Bachelor’s degree in Social Work, Counseling or related Human Service field preferred.
  • Certified Peer Recovery Specialist certification preferred or obtain CPR certification within six months.
  • 1-2 years of experience working in a behavioral health setting.
  • Knowledge of health care, detoxification process, addiction, co-occurring disorders, DSM Criteria, and terminology.
  • Ability to read and interpret written information; write clearly and informatively; edit work for spelling and grammar.
  • Ability to speak clearly and persuasively in positive or negative situations; listen and get clarification; respond well to questions; demonstrate group presentation skills; and participate in meetings.
  • Working knowledge of Microsoft Word, Excel, and Outlook.
