Job summary

Qualifications

• Bachelor’s degree in Social Work, Counseling or related human service field.
• One or more years’ experience working in a behavioral health setting preferred.
• Previous experience in case management and/or utilization review preferred.

Responsibilities

• Provide case management and care coordination for clients/patients/residents in the facility.
• Coordinate continuum of care activities and ensure optimal resource utilization and compliance with external agencies.
• Conduct psychosocial assessments and individual, family and group sessions.
