Job summary

Qualifications

• Bachelor's Degree in a related field preferred.
• Minimum of one (1) year of professional experience in Behavioral Health/Substance Use Treatment.
• Knowledge of health care, detoxification, addiction, co-occurring disorders, DSM criteria and terminology.

Responsibilities

• Completes and reviews biopsychosocial assessments and incorporates identified needs into the continued care plan.
• Develops and updates treatment plans with the multidisciplinary team and supports ongoing engagement in treatment.
• Completes release-of-information consents for identified supports and coordinates with patient ecosystem.
• Collaborates with patient and family to implement evolving continued care plans and follows referent protocols.
