Job summary

Boston Medical Center is seeking a Behavioral Health Complex Care Manager to identify and engage high-need clients for intensive care management across Boston, with in-person community visits and close collaboration with providers.

The role requires experience with Medicaid populations, strong interpersonal skills, and ability to manage a fast-paced workload while maintaining HIPAA standards and best practices in care coordination.

Qualifications

• Master's Degree in Social Work or Counseling.
• Experience providing case management services.
• Experience working with vulnerable patient populations and with serious mental illness/substance use disorders.

Responsibilities

• Identify and engage appropriate clients for complex care management.
• Perform comprehensive bio-psycho-social-spiritual assessments and develop care plans.
• Coordinate referrals to medical/behavioral health, housing, nutrition, transportation, and other services.
• Provide mobile in-community care management and participate in discharge planning.
• Maintain HIPAA standards and document progress and billing.
