Qualifications

• Bachelor's degree in a behavioral health field is required; a Master's is preferred.
• CCM credential is a plus but not required.
• Experience in case management, behavioral health, or related fields is required.

Responsibilities

• Coordinate patient care with primary care and behavioral health teams.
• Conduct behavioral health assessments and care plans.
• Maintain outreach and follow-up with high-risk patients.
• Provide psychoeducation to patients and families.
• Connect patients with community resources and social services.
• Coordinate referrals, authorizations, and follow-ups.
• Assist patients in goal setting and action planning.
• Document within the Electronic Health Record (EHR).
• Participate in trainings, meetings, and quality initiatives.
• Ensure compliance with HIPAA and organizational policies.
• Perform other duties as assigned.
