Job summary

ViaQuest Psychiatric & Behavioral Solutions in Wooster, OH seeks a Behavioral Health Case Manager to coordinate and deliver services within a multidisciplinary team. The role emphasizes planning, linkage to resources, and supporting individuals with mental health needs across age groups.

The position involves providing behavioral services, developing treatment plans, and promoting collaborative relationships with outside contacts. It is a full-time, 9am-5pm role in a client-centered setting.

Qualifications

• High school diploma plus 3+ years of relevant field experience or a bachelor's/master's in a related field.
• Related experience or training with mental or behavioral health conditions.
• Must have a valid driver's license and reliable transportation.

Responsibilities

• Provide Therapeutic Behavioral Services to clients by consulting with a licensed clinician to assist with the individual's needs.
• Provide service planning for individualized supports or care coordinator of healthcare, behavioral health, non-healthcare services and development of a treatment plan.
• Provide linkage, interventions, treatment options, restoration of daily life skills and crisis prevention services.
• Provide services to individual clients or groups of clients.
• Promote a positive and cooperative relationship with all outside contacts.

Education

$18.20-$20.90 based on education and experience plus monthly incentive program for exceeding productivity expectations

Requirements for this position include:

• Must be at least 18 years of age.
• High school diploma plus three or more years of relevant field experience or Bachelor's or Master's level degree in a related field.
• Related experience or training with mental or behavioral health conditions.
• Must have a valid driver's license and reliable transportation.
• Willingness to travel throughout assigned service area.
