Job summary

ViaQuest Psychiatric & Behavioral Solutions in Warren, OH is seeking a Behavioral Health Case Manager for full-time work Monday–Friday, 9am–5pm. Salary ranges from $18.20 to $20.90 per hour plus a monthly incentive for productivity, with a focus on therapeutic services and community supports.

The role involves creating tailored treatment plans, coordinating healthcare and behavioral health services, and maintaining strong collaboration with clients and outside contacts.

Qualifications

• High school diploma or higher with 3+ years of relevant field experience or a related degree.
• Willingness to travel throughout the assigned service area.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide Therapeutic Behavioral Services to clients by consulting with a licensed clinician to assist with the individual's needs.
• Provide service planning for individualized supports or care coordinator of healthcare, behavioral health, non-healthcare services and development of a treatment plan.
• Provide linkage, interventions, treatment options, restoration of daily life skills and crisis prevention services.
• Provide services to individual clients or groups of clients.
• Promote a positive and cooperative relationship with all outside contacts.

Requirements for this position include:

• Willingness to travel throughout assigned service area.
