The Behavioral Health Case Manager serves clients receiving co-occurring medical, mental, and behavioral health services at Bonneville Family Practice & Tooele Methadone Treatment. This position primarily supports adults receiving substance use disorder treatment, including individuals participating in our Opioid Treatment Program (OTP) and receiving medications for opioid use disorder (MOUD). Clients may have complex mental health, medical, housing, legal, transportation, financial, and other psychosocial needs.

This position is vital to the daily functioning of the program and works with a high-risk, high-need population. The Case Manager works collaboratively with behavioral health clinicians, medical providers, OTP staff, community agencies, and other members of the treatment team to reduce barriers to care and support treatment engagement, recovery, and stability.

Job Description and Responsibilities

• Performs 80% of paid hours in serving clients through direct case management and other designated productive/revenue-generating activities in accordance with organizational and payer requirements.
• Coordinate care with external agencies, family, payer sources, and other services as needed.
• Send monthly reports and updates for patients involved with courts, AP&P, AMP, and DCFS regarding treatment progress, UAs, and attendance when appropriately authorized and in accordance with confidentiality requirements and releases of information.
• Manage files and review relevant treatment-team documentation; maintain timely, accurate case management documentation in the electronic health record.
• Perform initial case management needs assessments to identify barriers, strengths, resources, and areas requiring care coordination.
• Create individualized care plans with measurable goals based on identified client needs and treatment requirements.
• Provide follow-up based on client need, acuity, care plan, and program requirements.
• Provide resources, information, and referrals as needed, including housing, transportation, food assistance, benefits, medical and mental health care, employment resources, and other community supports.
• Help coordinate placement for residential treatment and other appropriate levels of care, including detoxification and community-based treatment services.
• Participate in client-related meetings and trainings, including interdisciplinary case consultation and discharge/transition planning.
• Monitor and evaluate client progress through the case management plan and revise the plan as needed.
• Prepare case management reports documenting concerns, barriers, referrals, progress, successes, and outcomes.
• Enroll patients into external programs and assist clients with navigating Medicaid, public benefits, and community resources.
• Actively monitor approved client communication systems, including the texting program.
• Coordinate care that is safe, timely, effective, efficient, equitable, and client-centered.
• Handle case assignments and review case progress; prioritize high-risk, urgent, and time-sensitive needs.
• Help clients achieve wellness and program compliance by identifying and addressing barriers to treatment engagement and recovery.
• Respond to immediate case management needs when clients present for services and coordinate appropriate next steps.
• Take a proactive approach by identifying emerging needs and barriers before they become crises, initiating appropriate interventions and referrals, and engaging clients who may not independently seek assistance.
• Use active follow-through and warm handoffs when possible to help clients successfully connect with needed services rather than relying on referral information alone.
• Identify urgent safety, behavioral health, medical, or substance-use concerns and promptly escape them to the appropriate clinical or medical team member according to established procedures.
• Assist with transitions between levels of care, discharge planning, and continuity of care.
• Collaborate with therapists, medical providers, dosing/nursing staff, and outside providers to support coordinated treatment.
• Maintain professional boundaries, client confidentiality, HIPAA compliance, and appropriate releases of information.
• Provide trauma-informed, recovery-oriented, and client-centered support to individuals who may be actively using substances, ambivalent about treatment, or experiencing significant barriers to engagement.
• Participate in quality-improvement activities and support applicable organizational, Medicaid/payer, behavioral health, and OTP documentation and compliance requirements.

• Experience working with individuals receiving MOUD or within an Opioid Treatment Program preferred
• Familiarity with Medicaid-funded services and community resource navigation preferred
• Associate's or Bachelor's degree in behavioral health, psychology, social work, human services, sociology, or a related field preferred, or equivalent relevant experience/qualifications
