Forge Evolution is seeking a Behavioral Health Case Manager in Colorado Springs to support youth and families through case management, behavioral health support, and individualized interventions, working closely with the Executive Director and clinical team. Qualifications

Job description

The Behavioral Health Case Manager supports youth and families participating in Forge Evolution programs through case management, behavioral health support, individualized intervention, and family advocacy. This position works closely with the Executive Director, Program Manager, Program Coordinator, and Clinical Supervisor to help youth successfully complete programming while accessing the resources and support needed to improve long‑term outcomes.

This role serves as a primary liaison for youth and families, providing ongoing communication, behavioral health support, care coordination, and crisis intervention within a collaborative nonprofit environment focused on restorative justice, youth development, resiliency, and prevention services.

The Behavioral Health Case Manager may provide individual therapy, behavioral health assessments, and behavioral health interventions based on licensure, qualifications, and organizational needs.

This position reports to the Executive Director.

Youth & Family Case Management

• Maintain and manage an active caseload of youth participating.
• Conduct regular outreach and follow‑up to:
  • Review program and sentencing requirements
  • Identify barriers to completion
  • Assess behavioral health, educational, and family support needs
  • Provide encouragement, accountability, and guidance
• Serve as a primary family liaison for youth needing additional support or intervention.
• Support youth and families in understanding program expectations, timelines, and next steps.
• Attend IEP, school, or family support meetings as needed to advocate for youth success and coordinate services.
• Assist families in navigating community systems and accessing appropriate resources.

Behavioral Health Support & Clinical Collaboration (depends on licensure)

• Work collaboratively with the Clinical Supervisor to:
  • Assess individual youth needs and situations
  • Identify appropriate behavioral health interventions and supports
  • Connect youth to Behavioral Health Services (BHS), classes, counseling, or continued care
• Provide individual therapy and supportive interventions when clinically appropriate and within scope of licensure.
• Conduct check‑ins with youth struggling with substance use, behavioral concerns, or program compliance.
• Follow up with students who fail UAs to provide support, resources, encouragement, and intervention planning.
• Participate in case consultations and multidisciplinary discussions to ensure coordinated care.

Program Support & Facilitation

• Teach and facilitate youth development and prevention classes, including ART and ADAPT.
• Serve as backup facilitator for additional evening classes and program sessions as needed.
• Teach Teen Court classes as a prevention model in local schools including Life Skill, ART, ADAPT.
• Attend Peer Panels two Tuesday evenings per month.
• Provide backup support for Teen Court trial evenings when necessary.
• Assist in maintaining a safe, structured, and supportive environment for youth participants.

Documentation, Reporting & Compliance

• Maintain accurate, timely, and confidential client records and case documentation, including:
  • Progress notes
  • Behavioral health coordination notes
  • Resource referrals
  • Case management tracking
• Monitor participant progress in collaboration with the Program Coordinator and Program Manager.
• Assist with intake documentation, prevention referrals, and program enrollment processes.
• Ensure documentation complies with organizational, legal, and confidentiality standards.

Resource Coordination & Community Engagement

• Assist families with accessing community resources including behavioral health, housing, food assistance, education, medical care, and workforce support.
• Maintain collaborative relationships with schools, referral agencies, and community providers.
• Represent Forge Evolution professionally at community meetings, outreach events, and partner interactions.
• Assist with organizational events, fundraising activities, and operational support as needed.
• Full‑time position with varying hours based on program needs.
• Mandatory evening hours are required, including classes, Peer Panels, and occasional Teen Court sessions.
• Local travel to schools, court facilities, and community meetings may be required.
• Salary range: $60,000 – $70,000 annually, based on licensure and experience.

Preferred Qualifications

• Licensed or unlicensed clinician (LPC, LPCC, LSW, LCSW).
• Bachelor’s degree required; Master’s degree preferred in social work, counseling, psychology, behavioral health, criminal justice, or related field. Equivalent experience may be considered.
• Minimum of two (2) years of experience working with youth and families.
• Experience in behavioral health, case management, restorative justice, juvenile services, nonprofit programs, or community‑based youth work preferred.
• Bilingual abilities are a plus.

• Valid driver’s license and reliable transportation preferred.
