Job summary

Qualifications

• Bachelor’s degree in Social Work or related behavioral science field required; Master’s degree preferred.
• Minimum of 2 years of case management experience in behavioral health or outpatient settings.
• Must have own transportation and valid driver's license; local travel required.

Responsibilities

• Formulates and executes comprehensive discharge and transition plans to prevent readmission and ensure long-term stability across all ages.
• Serve as a bridge between outpatient mental health services, adult/child support networks, and welfare systems.
• Assess insurance coverage (Medicaid, Medicare, Commercial) and assist with enrollment and renewals; resolve coverage barriers.
• Maintain up-to-date knowledge of community supports and social determinants of health.
• Perform comprehensive social needs assessments for individuals, families, and youth; set measurable goals.
• Coordinate with emergency services and clinical teams for stabilization and referrals when needed.
• Maintain timely documentation in the EHR and provide service plans with monitoring of goals.
• Participate in supervision and perform other duties as needed.
