Job summary

Family Service Association of Bucks County seeks a Nurse Navigator to join our team in Langhorne, PA. This full-time nursing role focuses on care coordination, education, and advocacy to support clients with behavioral and physical health needs.

You will work across office, home, and community settings, coordinating services with psychiatrists, therapists, nurses, and case managers, while ensuring documentation and compliance.

Qualifications

• Valid Pennsylvania LPN or RN license required.
• Valid driver's license required.
• Experience in community health preferred.
• Proficient with HMIS, Credible, ADP and MS Office.

Responsibilities

• Provide nursing support and care coordination across office, home, and community settings.
• Perform medical assistant duties as needed to ensure coverage and care.
• Educate clients on medications, diagnoses, wellness, and harm reduction.
