Job summary

MaineGeneral Medical Center is seeking an Inpatient Behavioral Health Case Manager to provide comprehensive care management for inpatient behavioral health patients. You will assess needs, coordinate services, and support safe transitions of care as part of a multidisciplinary team.

The role features a day schedule with a 36-hour work week, no weekend requirement, and a focus on collaborative, patient-centered care within an inpatient psychiatric setting including detox scenarios.

Qualifications

• Experience in behavioral health, care management, or related clinical setting.
• Strong assessment, communication, and care coordination skills.
• Ability to work collaboratively within a multidisciplinary team.
• Patient-centered, compassionate approach to inpatient care management.

Responsibilities

• Interviews and assesses patients and families to identify clinical, psychosocial, and resource needs.
• Determines and arranges for appropriate internal and external services and interventions.
• Consults with multidisciplinary teams to evaluate status and progress of care.
• Performs chart reviews to address issues related to service delivery and outcomes.
• Participates in discharge planning and coordinates post-discharge resources.
• Provides discharge education and resource referrals to support safe transitions.
• Coordinates care for inpatient psychiatric patients, including detox patients.
