Job summary

Carson Valley Health in Gardnerville, NV is seeking a Care Coordinator for the OPBH & Vitality for Life program. The role centers on comprehensive care management, assessment, coordination, and patient advocacy to reduce barriers and promote recovery.

You will work with patients with complex behavioral health needs, collaborate with a multidisciplinary treatment team, and document in the electronic health record to ensure seamless transitions across care settings.

Qualifications

• Bachelor’s degree in a mental health or related field required.
• Master’s degree in mental health field preferred.
• Community Health Worker preferred.
• Licensed Master Social Worker preferred.

Responsibilities

• Conduct comprehensive biopsychosocial assessments to identify patient needs.
• Develop and update individualized care management plans with patients, families, and treatment team members.
• Identify high-risk and medically complex behavioral health patients who may benefit from enhanced care management.
• Coordinate transitions of care between outpatient, inpatient, emergency department, primary care, specialty care, and community-based services.
• Monitor patient progress toward treatment goals and intervene when barriers to care or treatment adherence are identified.
• Support patients in achieving improved health outcomes, increased functioning, and enhanced quality of life.
• Utilize evidence-based and recovery-oriented approaches to support patient engagement and self-management.
• Maintain accurate and timely documentation in the electronic health record.
