Washoe Tribe of Nevada & California is seeking a Manager of Medical Case Management in Gardnerville, NV. This exempt role coordinates chronic illness care, referrals, and records within the Washoe Tribal Health Center.

Under the Director of Nursing, the RN must hold licensure, have 3+ years in outpatient care, and obtain BLS/ACLS within 90 days. The manager will document meticulously, triage, and drive quality improvement in a collaborative clinical team.

Qualifications

• Current State RN license; NV licensure within 90 days for out-of-state applicants.
• Minimum 3 years’ experience in outpatient/ambulatory care.
• BLS and ACLS certifications within 90 days of hire.
• Must complete immunization and PPD testing per policy.
• Commitment to HIPAA and confidentiality; able to document meticulously.

Responsibilities

• Serve as Case Manager for patients with chronic illnesses; maintain registry and coordinate referrals.
• Schedule and track referrals, coordinating transportation as needed.
• Review labs, tests, prescriptions; ensure records are submitted for review.
• Maintain complete, accurate medical records and schedule follow-ups.
• Collaborate with physicians and nursing staff in patient care decisions.
• Provide triage by phone and in-office, assessing chief complaints and needs.
• Assist in policy development, quality improvement, and regulatory readiness.
