Job summary

Molina Healthcare, Inc. in Michigan seeks a licensed behavioral health clinician to support care management and coordination across the continuum of member care.

You will collaborate with a multidisciplinary team, develop and implement care plans, conduct visits (telephonic, in-person, or home), and monitor outcomes while traveling locally as required. Residency in Michigan is expected.

Qualifications

• 2+ years healthcare experience, preferably behavioral health.
• Active license as a behavioral health clinician in state of practice.
• Valid driver's license and reliable transportation for travel.
• Experience with severe mental health, SUD, or foster care.
• Proficiency with data entry and a clinical platform.

Responsibilities

• Complete comprehensive behavioral health assessments per regulated timelines.
• Develop and implement care plans with member and care team.
• Conduct telephonic, in-person, or home visits as required.
• Monitor care plan effectiveness and document progress.
• Maintain ongoing member caseload with regular outreach.
• Promote integration of behavioral health with LTSS and community resources.
• Facilitate interdisciplinary care team meetings.
