Job summary
Generation-Gaither in Cookeville, TN is seeking a Care Manager to coordinate services for individuals needing access to community resources, build natural supports, and facilitate care coordination among health providers under the Tennessee Health Link program.
The role focuses on creating individualized Person-Centered Care Plans, timely documentation, and collaboration with the Care Team to empower individuals to manage daily mental health needs and symptomology.
Qualifications
- Bachelor’s degree in health-related field or nursing license required.
- LPNs must have 15 semester hours in behavioral health and 1 year of behavioral health experience.
- Alternate Bachelor’s degree requires 15 college hours in behavioral health or 1 year of behavioral health work.
Responsibilities
- Care Management program provides services aligned with member rights, confidentiality and state/federal regulations.
- Encourage involvement of members and families in community resource development and linkages.
- Develop Comprehensive Care Plans within 24 hours of enrollment and review every 90 days.
- Maintain Crisis Plans within 24 hours of enrollment and document plan components monthly.
