Job summary

The Case Manager at Gateway CSB PEO LLC provides comprehensive care coordination, linking behavioral health, housing, employment, and community supports to improve access and engagement in treatment.

Using a person-centered, trauma-informed approach, you will advocate for clients, coordinate services across multiple systems, document activities, and participate in interdisciplinary teams to support recovery and community integration.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Counseling, Human Services, Sociology, Behavioral Health, Public Health, Healthcare Administration, or a related field.
• Valid Georgia Driver’s License.
• Ability to meet and maintain all position-related qualifications, training requirements, certifications, and competency standards.
• Preferred Experience in behavioral health, substance use disorder services, case management, care coordination, housing services, healthcare, or human services.

Responsibilities

• Care Coordination: Coordinates behavioral health, medical, housing, social, vocational, and support services for assigned consumers.
• Treatment Planning and Service Support: Participates in treatment planning and service coordination activities; assists consumers in achieving goals.
• Community Resource Development: Develops partnerships to improve service access; provides information about services and resources.
• Crisis Support and Care Transitions: Assists with crisis response and transitions between levels of care.
• Documentation and Compliance: Maintains documentation within required timeframes and ensures compliance with Medicaid, DBHDD, payer, and agency standards.
• Productivity and Quality Management: Maintains productivity standards and participates in quality improvement activities.
