Job summary

Cerula Care seeks a Behavioral Health Care Manager + Provider Success professional to lead on-the-ground behavioral health management with oncology partners in Colorado. The role blends clinical care management (80%) with provider success and field engagement (20%), delivering high-quality, relationship-centered care to patients while coordinating with oncology providers across RMCC locations.

The position is Colorado-based and hybrid, with travel to RMCC offices for site launches, trainings,

Qualifications

• Active, unrestricted CO state license in counseling or social work is required.
• Minimum three years of clinical care management experience, including work with cancer, serious illness, or chronic conditions.
• Experience conducting behavioral health intakes, psychosocial assessments, safety screens, and treatment planning.

Responsibilities

• Lead clinical care management activities (intakes, assessments, safety screens, treatment planning, documentation).
• Provide follow-up visits and brief, evidence-based interventions.
• Build relationships with patients and engage via telehealth and in-person visits.
• Coordinate with the BH team, psychiatrist, and care partners; document progress in HIPAA-compliant EMR.
• Facilitate referrals and communicate concerns to providers and psychiatrists.
• Serve as liaison to RMCC providers and office teams; travel to Colorado sites for launches/trainings.
