Responsibilities

• Provide care coordination for ECM patients with behavioral health needs.
• Perform brief crisis interventions and warm hand-offs to crisis resources.
• Educate patients/families to improve health literacy and adherence.
• Coordinate referrals and appointments with PCPs and specialists.
• Document care coordination activities in the EHR (eClinicalWorks).
• Engage in weekly case reviews with ECM team and supervisors.
