Qualifications

• Bachelor's or RN with health care/human services background
• Master’s degree in healthcare/human services is highly encouraged
• Two years of clinical experience in focused areas with multidisciplinary teams
• Valid driver’s license for work-related driving

Responsibilities

• Utilization Management: monitors length of stay, level of care changes for compliance and documents the case management plan.
• Care Coordination/Discharge Planning: assesses health status, coordinates interdisciplinary plan, ensures timely and safe transition across care settings.
• Revenue Cycle: understands financial and reimbursement processes and educates patient and family on benefits and resources.

Education

Bachelor or RN with health care/human services background
