Schedule: Full-time | Monday-Friday 8am-4:30pm
Assess and Evaluate Patients: Conduct comprehensive behavioral health assessments for hospital patients to identify mental health conditions, substance use disorders, and immediate care needs.
Develop Therapy Plans: Formulate personalized, evidence-based treatment and therapy plans in collaboration with the multidisciplinary medical team.
Provide Crisis Intervention: Deliver immediate crisis intervention and stabilization for patients experiencing acute distress, including rigorous risk assessments for suicidal ideation.
Deliver Health Coaching: Offer tailored, one-on-one behavioral health coaching to support patients in developing coping strategies and achieving long-term wellness goals.
Licensure / Certification / Registration:
BLS Provider preferred. American Heart Association or American Red Cross accepted.
Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.
Counselor preferred.
Social Worker preferred.
Education:
Personalized behavioral health coaching and crisis intervention experience a plus
Degree in Social Work or Psychology a plus
